We are seeking a dedicated Licensed Vocational Nurse (LVN) for a Long Term Care assignment in Morgan Hill, California . Key Responsibilities
-
Provide general nursing and psychiatric care within LVN scope of practice.
-
Deliver direct resident care, behavior support, and supervision.
-
Implement and support individualized treatment plans and behavioral programs.
-
Participate in interdisciplinary care planning and team collaboration.
-
Oversee DSPs and CNAs when functioning as lead staff.
-
Facilitate structured programming and therapeutic interventions throughout assigned shifts.
-
Maintain accurate documentation and adhere to established care protocols.
-
Report concerns and incidents in accordance with mandated reporter guidelines.
-
Support a safe, therapeutic, and resident-focused environment.
Qualifications
Required
-
Current, active California LVN license.
-
Experience working with adults with Intellectual and Developmental Disabilities (IDD).

Strong communication and teamwork skills.

Ability to follow treatment plans and behavioral support strategies.

The Long Term Care LPN assumes responsibility and accountability for the delivery of care for patients in a Long Term Care/Assisted Living Facility. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
Minimum Requirements:

Current Licensed Practical Nurse License for the state in which the nurse practices

Current Health Certificate (if applicable)

Current PPD or Chest X-Ray

One year prior professional nursing experience preferred

Current BLS card
-
Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater

Successful completion of Background Screening Process
