Full-Time LPN Case Manager - Brockton, MA
Location: In-home assessments throughout Brockton and surrounding towns
Schedule: Monday-Friday | 9:00 AM - 5:00 PM
Assignment: 13 weeks to start (Temp-to-Perm & Direct Placement options available)

Position Overview
Amergis Healthcare Staffing is partnered with a respected Adult Day Health organization dedicated to providing high-quality, community-based care for elderly adults and individuals with disabilities. Their mission is to support clients and caregivers in maintaining safety, comfort, and independence through personalized care plans, ongoing clinical oversight, and proactive case management.
As an LPN Case Manager, you'll play a key role in evaluating client needs, supporting caregivers, updating care plans, and ensuring continuity of services. This position offers meaningful one-on-one interactions, clinical autonomy, and the opportunity to make a direct impact in the community.
Key Responsibilities

Complete intake, admission, and annual assessments in collaboration with nursing staff

Conduct monthly in-home visits, provide caregiver education, and assess psychosocial and environmental changes

Perform semi-annual visits to review and update care plans and goals with nursing

Evaluate the appropriateness of care settings and caregiver support systems

Assist clients with accessing healthcare and community resources

Participate in multidisciplinary care planning, case reviews, and discharge planning

Address concerns related to home environment and client well-being

Collaborate with internal teams and external providers to ensure continuity of care

Maintain accurate and timely documentation in compliance with regulations

Communicate professionally with clients, caregivers, PCPs, and healthcare providers

Support a culture of high-quality care and customer service
-
Perform additional duties as assigned
Qualifications

Active LPN license in good standing with the Massachusetts Board of Registration in Nursing

Minimum 2 years of recent experience working with elderly populations or individuals with disabilities

Valid driver's license

Bilingual (English + another language) preferred

Experience in home health, case management, or community-based care preferred
Working Conditions

Combination of field-based and office work

Frequent travel to client homes and community settings

Exposure to varying home environments (temperature changes, pets, allergens, etc.)

Prolonged computer use and documentation

Ability to lift, carry, push, or pull up to 35 pounds

Requires walking, standing, sitting, and navigating stairs
