We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to provide skilled one-on-one nursing care to patients in their homes throughout St. Tammany, Washington, and Tangipahoa Parishes . This position offers the opportunity to make a meaningful impact while working independently in a rewarding home health environment.
Under the supervision of a Registered Nurse, the LPN will deliver direct patient care, provide education to patients and families, and assist with a variety of clinical needs in the home setting.
Responsibilities

Provide skilled nursing care in patients' homes

Perform wound care and dressing changes
-
Manage drains, ostomies, and surgical incisions

Collect and process lab specimens as required

Educate patients and caregivers on disease management and treatment plans

Accurately document care using Homecare Homebase (HCHB)

Monitor patient conditions and report changes to the supervising RN
-
Maintain compliance with all regulatory and clinical standards

Travel between patient homes within assigned coverage areas
Required Qualifications
-
Graduate of an accredited Practical or Vocational Nursing program
-
Current Louisiana LPN license in good standing

Minimum one year of professional nursing experience

Home Health experience required
-
Experience with Homecare Homebase (HCHB) required

Current CPR certification
-
Current TB screening (questionnaire, PPD, or chest X-ray as applicable)

Current health certificate (per state regulation or contract requirements)

Must meet all federal, state, and local employment requirements

Must be at least 18 years of age
Preferred Qualifications

Strong patient education skills
-
Experience managing wound care, drains, ostomies, and post-surgical patients

Excellent communication and documentation skills
-
Ability to work independently and manage a mobile patient caseload
