Licensed Practical Nurse (LPN) Case Manager
Location: Northeast MA (Reading, Lowell, Lawrence, Salem area)
Schedule: Monday-Friday, 9:00 AM-5:00 PM
Type: Full-Time | 13-Week Contract (Temp-to-Perm or Direct Hire)
Pay: $47/hour + $0.72/mile reimbursement
Position Overview
Amergis Healthcare Staffing is partnering with an Adult Day Health organization seeking an LPN Case Manager to support elderly adults and individuals with disabilities in the community. This role focuses on client assessments, care plan management, caregiver support, and care coordination through regular home visits.
Key Responsibilities

Complete intake, admission, annual, and ongoing client assessments.

Conduct monthly home visits and provide caregiver education.

Review and update care plans in collaboration with nursing staff.

Coordinate services and connect clients with healthcare and community resources.

Participate in interdisciplinary care planning and discharge planning.

Maintain accurate documentation and ensure continuity of care.

Communicate with clients, caregivers, and healthcare providers.
Qualifications

Active Massachusetts LPN license in good standing.

2+ years of experience working with elderly adults or individuals with disabilities.

Valid driver's license and reliable transportation.

Home health, case management, or community-based care experience preferred.

Bilingual candidates strongly preferred.
