Licensed Practical Nurse (LPN/LVN) - Home Health
Location: Grants Pass, Oregon
Sign-On Bonus: $10,000
Schedule: Full-time, Monday-Friday
Remote: No; field-based position

Responsibilities

Provide skilled nursing visits in patients' homes according to physician-prescribed plans of care

Monitor patients' conditions and communicate changes to the appropriate clinical team members

Educate patients and caregivers on medications, treatments, and ongoing care needs

Complete accurate, timely, and thorough clinical documentation

Collaborate with physicians, nurses, therapists, and other members of the care team

Travel throughout the assigned coverage area to support patients in their homes
Qualifications

Graduate of an accredited practical or vocational nursing program
-
Active Oregon LPN or LVN license
-
At least one year of clinical nursing experience

At least one year of home health experience
-
Current CPR certification
-
Valid driver's license and ability to travel throughout the assigned territory

Strong clinical judgment, communication, and documentation skills
