How you'll make an impact in this role

Coordinate holistic care plans by partnering across multidisciplinary teams to deliver seamless, well-managed care journeys for every patient.

Monitor and communicate patient progress in real time to ensure prompt medical interventions and keep families informed.

Execute physician and nursing orders with precision to maintain the highest standards of safety and treatment accuracy.

Assess and drive discharge planning alongside the healthcare team to prepare patients and their families for a smooth, confident transition home.
What minimum requirements you'll need
Licensure/Certification/Registration:

BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date required.

Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or jobtransfer date required.
Education:
Work Experience:
- RN with approximately 18 months or more experience preferred.
