Contract-to-Hire Opportunity
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our primary care team in Putnam, Connecticut. This position offers the opportunity to work closely with providers, patients, and care teams to deliver exceptional healthcare services in a collaborative outpatient setting.

Responsibilities

Triage incoming patient calls and assess symptoms, urgency, and appropriate next steps

Room patients and obtain vital signs prior to provider visits

Provide direct patient care within the RN scope of practice

Document patient assessments, interventions, and communications in the EMR system

Maintain accurate and timely patient charting

Schedule appointments, referrals, and follow-up visits

Assist with chronic disease management and care coordination initiatives

Educate patients regarding treatment plans, medications, and preventive care

Collaborate with providers and clinical staff to support quality patient outcomes

Ensure compliance with clinical protocols and patient confidentiality requirements
Required Qualifications

Active Connecticut Registered Nurse (RN) License

Active BLS Certification prior to start date

Experience triaging patient concerns in a clinical setting

Experience rooming patients and obtaining vital signs

Electronic Medical Record (EMR) experience

Strong assessment, documentation, and communication skills

Ability to thrive in a fast-paced outpatient environment
Preferred Qualifications

Previous experience in Primary Care, Family Practice, Community Health, or Outpatient Clinics

Experience with TS Energy EMR or similar systems

Care management and patient education experience

Strong organizational and multitasking abilities

Job Type & Location
This is a Contract to Hire position based out of Putnam, CT.
Pay and Benefits
The pay range for this position is $35.00 - $40.00/hr.
Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
