As a Registered Nurse in our primary care setting, you will play a vital role in coordinating patient care, performing clinical assessments, supporting chronic disease management, and helping patients navigate their healthcare journey. Key Responsibilities

Triage patient phone calls and clinical inquiries, assessing symptoms and determining the appropriate level of care using clinical judgment and established protocols.

Provide direct nursing care and clinical support within the scope of RN practice.

Room patients efficiently by verifying information, collecting vital signs, and preparing them for provider visits.

Document patient assessments, care plans, interventions, and communications accurately within the electronic medical record (EMR) system.

Coordinate patient appointments, referrals, and follow-up care to ensure continuity of services.

Support care management initiatives, including chronic disease monitoring, preventative care outreach, and patient education.

Collaborate closely with providers, medical assistants, and interdisciplinary team members to deliver comprehensive, high-quality care.

Educate patients and families on treatment plans, medications, wellness strategies, and available community resources.

Maintain a safe, organized, and welcoming clinical environment focused on exceptional patient experiences.

Advocate for patients by helping address barriers to care and supporting positive health outcomes.
Required Qualifications

Active Connecticut Registered Nurse (RN) license.

Current Basic Life Support (BLS) certification prior to start date.

Experience performing patient triage in a clinical, outpatient, or primary care setting.

Strong assessment, critical thinking, and care coordination skills.

Experience utilizing electronic medical record (EMR) systems.

Excellent communication, documentation, and interpersonal skills.

Commitment to delivering compassionate, patient-centered care.
Preferred Qualifications

Previous experience in primary care, family medicine, community health, or outpatient healthcare settings.

Experience supporting chronic disease management and preventive care initiatives.

Familiarity with care coordination, patient outreach, and population health programs.

Ability to effectively manage multiple priorities in a fast-paced clinical environment.

Bilingual language skills are a plus.

This is a Contract to Hire position based out of Woodstock, CT.
Pay and Benefits
The pay range for this position is $35.00 - $40.00/hr.
Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
