Job Description:

Formulates individualized care plans based on therapeutic principles, evaluates the effectiveness of interventions, and modifies plans as needed to achieve desired patient outcomes.

Initiates patient coordination at the entry point into the system and prepares patient cases for pre-procedural review with a multidisciplinary team.

Navigates neuro medical inpatients from admission through treatment and discharge, ensuring timely medical and supportive services.

Assists in program development and expansion, actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Serves as a patient advocate, ensuring patient rights and needs are respected and addressed.
