RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:

Under the general direction of the Private Duty Manager, Branch Coordinator and/or Clinical Coordinator, performs skilled nursing services to clients/patients within established policies, procedures, and regulations, in accordance with the client's individualized plan of care in the client/patient's home.

Assesses, plans, implements, and evaluates nursing and other related services provided to clients.
II. TYPICAL PHYSICAL DEMANDS:
-
Sitting, finger dexterity, seeing, hearing, speaking.

Constant: reaching above, at and below shoulders; repetitive arm/hand motion.

Frequent: standing, walking, stooping/bending, gripping of objects, and twisting body.
-
Occasional: lifting and carrying items up to usual weight of 36-50 pounds, pushing/pulling 50 pounds of force.
III. TYPICAL WORKING CONDITIONS:
-
Some exposure to communicable diseases or bodily fluids; some toxic substances and other conditions common to health care and home environments.

Variable work sites.

Available on weekends, holidays and various available shifts should the minimum hours decrease.

Use of personal smart phones for business clinical documentation, communication, and access to work schedules.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
-
Bachelor's degree in Nursing.
-
Current Hawaii State license as a Registered Nurse.
-
Valid U.S driver's license, vehicle and vehicle insurance as required by Hawaii State Law.
-
Driver's abstract reflecting evidence of good driving record.
-
BLS Current certification.
B. PREFERRED EXPERIENCE:
-
Minimum of one (1) year recent experience as a registered nurse in an acute, home care or community setting.

Recent experience in home health.
-
Ability to chart/document on electronic device.
-
Skill in establishing and maintaining effective working relationships with clients, staff, and the public.
