Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) for a Travel Home Health contract in Madison, OH . This is a contract assignment providing home health case management services throughout Lake, Geauga, and Ashtabula Counties.

Job Details
· Location: Madison, OH
· Pay: $53-$ 55.90/hour
· Mileage: Reimbursed between patient visits
· Contract: Two to Three Month contract
· Schedule: Monday-Friday, 8:00 AM-4:30 PM
· On-Call/Weekend: One week of on-call coverage, including a weekend
· Coverage Area: Lake, Geauga, and Ashtabula Counties
Responsibilities
· Manage a home health patient caseload throughout the assigned service area.
· Assess patient needs and coordinate individualized plans of care.
· Monitor patient progress and communicate changes in condition to physicians and the healthcare team.
· Educate patients and caregivers on treatment plans, medications, and disease management.
· Maintain accurate and timely clinical documentation.
· Travel between patient visits throughout the coverage area.
· Participate in the required on-call rotation.
Requirements
· Active Ohio Registered Nurse (RN) license.
· Ability to work independently and manage a patient caseload.
· Valid driver's license and reliable transportation.
· Strong communication, organization, and documentation skills.

Pay Details: $53.00 to $55.90 per hour
