MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Wage: $18.00/hour
Schedule: Full-Time | Monday-Friday | 7:00 AM-3:30 PM

Qualifications

Active Certified Nursing Assistant (CNA) certification required.

High school diploma or GED required.

Minimum 1 year of CNA experience preferred, ideally supporting older adults or individuals with disabilities.

Current CPR/First Aid certification required.

Bilingual in English and Spanish preferred.

Comfortable assisting with personal hygiene, mobility, and daily living activities.

Ability to safely perform physical duties, including lifting and assisting participants.

Strong communication skills, compassion, and attention to detail.

Ability to travel as needed.

What You'll Do

Provide hands-on personal care, including hygiene, grooming, toileting, mobility, and daily living assistance.

Monitor participant health and safety, assist with medications as authorized, and respond to changes in condition or emergencies.

Support daily activities, social engagement, meals, and maintaining a clean, safe environment.

Maintain accurate documentation and collaborate with nursing staff to provide compassionate, person-centered care while ensuring confidentiality.
