Schedule: 8:00 AM-4:30 PM, 32 hours per week guaranteed

Coverage Area: Petersburg, WV and surrounding area

Mileage: Reimbursed between patient visits
Responsibilities
· Provide compassionate hospice care to patients in their homes.
· Assess patient conditions and support comfort, symptom management, and quality of life.
· Perform hospice needs assessments and assist with patient admissions.
· Manage patient cases and coordinate individualized plans of care.
· Participate in interdisciplinary group (IDG) meetings to discuss patient needs and care plans.
· Monitor changes in patient conditions and communicate with physicians and the healthcare team.
· Provide education and support to patients and their families.
· Maintain accurate clinical documentation using Homecare Homebase (HCHB).
· Travel between patients' residences throughout the Petersburg, WV area.
Requirements
· Active RN license or compact nursing license.
· Associate Degree in Nursing (ADN) or higher.
· Home health and/or hospice nursing experience.
· Experience with Homecare Homebase (HCHB).
· Strong communication skills and compassionate bedside manner.
· Ability to provide patient-centered care in a home setting.
· Ability to travel throughout the assigned coverage area.
· Availability to work the required schedule and discuss on-call/weekend coverage as needed.

Pay Details: $53.00 to $55.90 per hour
