Registered Nurse (RN) Case Manager - Home Health | Direct Hire
Position Overview
We are seeking a compassionate and skilled Registered Nurse (RN) Case Manager to provide home health services to patients in Watkinsville, GA and surrounding areas . This is a full-time, direct-hire opportunity offering a competitive salary and comprehensive benefits package.
Salary: $85,000 - $95,000 annually
Schedule: Monday - Friday

Key Responsibilities

Conduct comprehensive patient assessments and develop individualized plans of care

Provide skilled nursing services in accordance with physician orders and established care plans

Coordinate patient care with physicians, caregivers, and interdisciplinary healthcare team members

Monitor patient progress and recommend appropriate changes to treatment plans

Educate patients and families regarding disease management, medications, treatments, and home care needs

Maintain accurate, timely, and compliant clinical documentation
-
Ensure delivery of high-quality, patient-centered care in the home environment

Participate in ongoing care coordination and case management activities
Required Qualifications

Graduate of an accredited school of professional nursing
-
Active Registered Nurse (RN) license in the State of Georgia

Minimum of one (1) year of RN experience
-
Current CPR certification
-
Valid driver's license, reliable transportation, and current auto insurance

Ability to travel throughout the assigned coverage area

Previous home health , case management, or community-based nursing experience

Strong assessment, care planning, and patient education skills

Excellent communication and organizational abilities

Ability to work independently while managing a patient caseload

Ideal Candidate
The ideal candidate is a compassionate and organized Registered Nurse who is passionate about delivering quality care in the home setting. This individual will excel at building relationships with patients and families, coordinating care across disciplines, and promoting positive patient outcomes while working independently in the field.
Pay Details: $85,000.00 to $95,000.00 per hour
