Responsibilities:

Perform patient assessments and develop individualized care plans
-
Provide skilled nursing care in patients' homes
-
Educate patients and families on disease management and treatment plans
-
Coordinate care with physicians, caregivers, and interdisciplinary team members
-
Complete timely and accurate documentation
-
Help patients remain safe, healthy, and independent in their homes
Experience and Skills Needed:
-
Active RN license
-
Home Health experience required
-
OASIS experience strongly preferred
-
Strong clinical assessment and communication skills
-
Ability to work independently and manage a field-based schedule
