Job Title: Registered Nurse - Home CareJob Description
This Registered Nurse role provides skilled nursing services to patients in their homes, supporting their independence and quality of life. You assess patient needs, develop and implement individualized care plans, coordinate with physicians and caregivers, and monitor patient progress. You ensure that all care complies with agency policies and applicable state and federal guidelines in a home care environment.
Responsibilities

Provide skilled nursing care to patients in their homes in accordance with established care plans and professional standards.

Conduct comprehensive initial assessments of patients' physical, emotional, and social needs.

Perform ongoing patient assessments to evaluate response to treatment and adjust care as needed.

Develop individualized plans of care based on assessment findings and patient goals.

Implement and update care plans to reflect changes in condition, treatment, or patient preferences.

Educate patients and family members on disease management, medications, treatment plans, and self-care strategies.

Coordinate care with physicians, therapists, home health aides, and other healthcare professionals to ensure continuity and quality of care.

Communicate effectively with caregivers and interdisciplinary team members regarding patient status and care needs.

Monitor patient outcomes, identify changes in condition, and report significant findings promptly to the appropriate providers.

Ensure compliance with New York State Department of Health regulations and agency policies in all aspects of care delivery.

Complete clinical documentation accurately, thoroughly, and in a timely manner in medical records and related systems.

Engage in case management activities, including planning, organizing, and overseeing services to meet patient needs.

Support a patient-centered approach that promotes safety, independence, and quality of life in the home setting.
Essential Skills

Active New York State Registered Nurse (RN) license.

Associate's Degree in Nursing required.

Current BLS/CPR certification.

Demonstrated experience providing patient care in a healthcare setting.

Strong assessment skills to evaluate patient condition and needs.

Strong verbal and written communication skills to interact with patients, families, and healthcare team members.

Accurate and timely clinical documentation skills in medical records.

Ability to manage multiple patients and coordinate care effectively (case management).

Ability to ensure compliance with agency, state, and federal guidelines.

Bilingual Spanish communication skills.
Additional Skills & Qualifications

Bachelor of Science in Nursing (BSN) preferred.

Experience in home care, community health, acute care, or case management preferred.

Valid driver's license and reliable transportation preferred for field visits.

Experience working within managed care or similar healthcare delivery models.

Familiarity with healthcare regulations and New York State Department of Health requirements.

Ability to educate patients and families on disease processes, medications, and treatment plans.

Strong organizational skills and attention to detail in planning and delivering care.

Ability to collaborate effectively within an interdisciplinary team.

Comfort working independently in a community and home-based setting.
Work Environment
This role is based in a home care environment, providing nursing services in patients' homes and the community rather than in a traditional facility. You travel to patient residences for field visits, using reliable transportation to reach various locations. The position involves working in diverse home settings, adapting to different household environments while maintaining professional standards of care and safety. You use standard healthcare equipment and medical supplies appropriate for home care and complete clinical documentation in medical records in accordance with agency procedures. Hours may require flexibility to meet patient needs, and you work collaboratively with an established home care provider focused on supporting patient independence and quality of life.

The pay range for this position is $104000.00 - $120000.00/yr.
