Qualifications:

Valid NY State RN license with a BSN degree

Proven experience in Intake or CHHA roles

Strong knowledge of the Home Care Homebase (HCHB) system

Familiarity with Allscripts and Careport platforms

Excellent communication and organizational skills

Compassionate approach to patient care and coordination
