What You'll Be Doing
-
Conduct and review Community Health Assessments using the NYS Uniform Assessment System (UAS)
-
Assist physicians in completing required orders and documentation
-
Perform in-home and virtual follow-up assessments
-
Review and validate assessment outcomes using clinical expertise
-
Recommend long-term care services and supports based on patient needs
-
Collaborate with interdisciplinary teams to address care gaps and improve quality outcomes
-
Provide clinical guidance and educational support for program initiatives
-
Utilize strong communication skills to effectively engage diverse patient populations
Qualifications
Active New York State RN License
Experience with UAS-NY and/or OASIS Community Assessment Tools
Minimum of 2 years of in-home healthcare experience within the last 15 years
Experience working with:
Strong computer and telehealth skills
Excellent written and verbal communication abilities
Knowledge of long-term care services and programs
Ability to travel throughout the assigned Brooklyn region for in-home visits

Pay Details: $55.00 to $58.00 per hour
